Here for you - Things you might like to discuss with the palliative care team (QUT)

Here for You is a practical guide designed to help families prepare for conversations with the palliative care team, including writing down questions, concerns or topics they may wish to discuss. The resource also brings together information about financial assistance, counselling and emotional support, respite services, education support, accommodation, community services and bereavement support.

Families are encouraged to speak with their primary medical care team and palliative care team about available supports and local services that may meet their needs.
