Risky Business? A Rapid Discharge Program approach to getting children home for end-of-life care

The Rapid Discharge Program (RDP) has been developed, tested, and proven to be highly effective in the adult palliative care sector to support patients dying at home. To align our paediatric service with current practice we modified the RDP framework and policies. We will present two vignettes that highlight the importance of using a screening tool to assess risk.
